Attach all relevant documents needed, with the NMS Application Form. The list of documents needed is written clearly on the Form itself.

(payable at Ahmedabad) for Rs 500=00 (Rs Five Hundred only), along with the Application Form.(If you are personally submitting the Form at NMS Ahmedabad, then you may pay in cash also at the counter. Please collect the Receipt.)
